most of the leg work for you until your action plan is completed.
Once an assessment has been completed identifying your specific needs;
an action plan and implementation plan will be completed. The action plan
identifies what steps need to be completed and when and who is responsible
for each task. Then the Home Benefits Consultant will manage each step
of the action plan to make certain each step is completed sequentially and
successfully. You can be involved as much as you like or as little as you like.
